The standard format for each publisher gives: main address and phone; order address; prepayment requirement; discount-, return-, shipping, billing, and backorder-policies; and standing order/approval plans. These statements define the minimum discounts/terms libraries can expect; the maximums are negotiable. Ugly bookmaking -- typescript, no running heads, acres of white space (the book would fit on less than 200 octavo pages if it were typeset -- and at a great gain in legibility and handiness).
